How do I find out if I’m eligible?

Find out if you may be eligible for the Disability Tax Credit by answering questions from the following self-assessment questionnaire.

This questionnaire does not replace the form itself. It is provided to help you assess whether you may be eligible for the Disability Tax Credit.

Self-assessment questionnaire

1. Has your impairment lasted, or is it expected to last, for 12 continuous months?

2. Are you blind *?

yes / no

3. Do you receive life-sustaining therapy *?

yes / no

4. Do the effects of your impairment cause you to be markedly restricted * in one of the following basic activities of daily living?

yes / no

5. Do you meet the cumulative effect * criteria?

AND

yes /no

Note: The cumulative effect of these significant restrictions is equivalent to being markedly restricted * in a single basic activity of daily living.

 

How do I interpret my answers to the self-assessment questionnaire?

If you answered “yes” to Question 1 and any one of Question 2 to Question 5, you may be eligible for the Disability Tax Credit. To apply for the Disability Tax Credit, complete page 1 of Form T2201, Disability Tax Credit Certificate. Then, take the form to a qualified practitioner * who can certify the effects of the impairment for you. If the qualified practitioner certifies the form, send it to your Canada Revenue Agency (CRA) tax centre for approval. The CRA will review your application and notify you in writing of their decision.

If you answered “no” to questions 2 to 5, you are not eligible for the Disability Tax Credit. For you to be eligible for the Disability Tax Credit, you have to answer “yes” to at least one of these questions.
